Dear Jeffrey,

 

Congratulations! The Arc Lexington is pleased to make you this conditional offer of Full-Time employment as a Direct Support Professional working Days, at a starting rate of pay of $17.10. Your rate of pay may change once you are accepted into a permanent assignment. You are eligible to apply for a permanent assignment upon satisfactory completion of all required training. This offer of employment is contingent upon our receipt of satisfactory results of all required pre-employment screenings.

 

We have scheduled your pre-employment screening appointment on 10/30/2023 at 9am. Upon arrival, please advise the receptionist you are there for pre-screening and she will direct you to the gymnasium on the first floor. Representatives from Human Resources and the Training Department will be on hand to guide you through the entire pre-screening process. Upon successful completion of the employment pre-screening, your training start date will be 11/6/2023.

 

It is important that you bring all the following items with you to your scheduled pre-screening appointment. Failure to provide the necessary information may delay your employment start date.

 

  • Driver's License/Permit/State Issued ID or Passport.

 

  • Acceptable documentation to verify identity and employment authorization for completion of the I-9. A list of acceptable documents is available by clicking the link below.

I-9 Acceptable Forms

 

  • Names of everyone living in your household and their date of birth.

 

  • Seven (7) years of employment history and ALL employment/volunteer work history (even beyond 7 years) for any position held where you assisted individuals with developmental disabilities.

 

  • ALL street addresses, including house or apartment numbers, city, state, and zip codes of places you have lived since 1992. You must know the month and year you moved in and the month and year you moved from each location. If you were born after 1994, you must include information going back to your date of birth. PO Boxes are not allowed. You must know the physical address.

 

  • Names of two (2) emergency contacts and their phone numbers.

 

  • You will receive the first of 2 required TB shots. The second TB shot must be completed within the first 30 days of employment. If you have experienced a prior significant reaction to TB testing, currently undergoing, or have completed preventative treatment, you must provide documentation of this from your healthcare provider. In addition, if you are pregnant, you must provide documentation from your healthcare provider that you can have the TB test.

 

  • You will have your picture taken for your Employee ID card that will be issued when you start employment.
